Medical Record Requests

Tri-County Behavioral Healthcare is committed to protecting the privacy and confidentiality of the individuals we serve while providing appropriate and timely access to records in line with state and federal privacy regulations.

How do I request medical records?

Requests for medical records may be submitted electronically to the Tri-County Medical Records Department at medicalrecords@tcbhc.org . Your medical record request will be processed and a notification will be sent via mail or email once complete and available for download.

Who can I contact if I have questions?

If you have questions or need to discuss options for requesting or receiving records you may contact the medical records department by calling (936) 521-6100 or sending an email to medicalrecords@tcbhc.org .

Public Information Act Requests

Under the Texas Public Information Act, members of the public may request access to certain records maintained by our organization.

Public information requests are not the same as medical records requests. If you are seeking your own medical or behavioral health records, or records you are legally authorized to obtain on behalf of another person, please follow our Medical Records Request process rather than submitting a Public Information Act request.

How to Submit a Public Information Request

A request may be made by submitting a written request with attention to the Tri-County Behavioral Healthcare Public Information Officer, by hand, to the main Conroe Facility at 233 Sgt. Ed. Holcomb Blvd. S. or by one of the methods listed below:
